What to Bring

To ensure that your first appointment goes as smoothly as possible, please bring the following information with you to your initial consultation:

• A list of the medications you are currently taking
• Your medical history
• Your medical and/or dental insurance information (if applicable)
• Your referral form and any X-rays you were given
• Your referring dentist or doctor may have sent your referral form and X-rays to us ahead of time. If you are unsure whether you need to bring a referral form, please contact our office before your appointment.

Please note: A parent or guardian must accompany all patients under the age of 18 at every oral surgery visit, including the initial consultation appointment.

What to Expect

Thank you for choosing MOSA Oral Maxillofacial & Dental Implant Surgery for your upcoming appointment!

The first visit to our office will consist of an oral examination and a thorough medical history review with Dr. Lewis, Dr. Lloyd, or Dr. LaMothe. We will also complete any necessary 3D imaging to diagnose your oral health problems and create a custom treatment plan to address them. We may perform your procedure on the same day as your consultation, but this is only the case for simple treatments.

3D Imaging

Our practice is fully digital, featuring cutting-edge CBCT scanning, digital X-rays, digital impressions, intraoral scanners, and advanced guided implant placement technology.

Financial & Insurance Information

MOSA Oral Maxillofacial & Dental Implant Surgery accepts cash, checks, Visa, MasterCard, Amex, and Discover. In some circumstances, patients may also qualify for financing through a third party, such as Care Credit. We deliver the finest care at the most reasonable cost to our patients; therefore, payment is due at the time service is rendered unless you make other arrangements in advance.

Dr. Michael Lewis, Dr. Ryan Lloyd, and Dr. Andre LaMothe provide you with premium surgical care and the most convenient financial options. To accomplish this goal, our team works hand in hand with you to maximize your insurance reimbursement for covered procedures. Please bring your insurance information with you to the consultation so that we can expedite reimbursement. You are fully responsible for all fees charged by this office regardless of your insurance coverage.
